Main Duties and Responsibilities
- To provide hands on care/re-enablement to patients within their own home by following a prescribed care/rehabilitation plan adhering to SMART goals.
- To work independently within a patients home or within any area of the Oxford University Hospital Sites supported by regular contact with an Assessor.
- To report changes or progress to an Assessor and participate in reviewing care plans.
- To Understand and contribute to risk assessments along with health and safety assessments and including immediately reporting any changes/ newly identified risks.
- To keep accurate and up to date records and data as identified by the Service Management Team (Head of Service / Service Manager/Assessor Coordinator etc).
- To ensure appropriate procedures are followed in the case of any accidents or incidents through Ulysees.
- To complete, record and monitor records whether paper or electronic, assessing individual patient needs and escalate where appropriate. To complete all Service documentation associated with Service allocated patients.
Specific Responsibilities
1.
Communication
1. Provide and receive information to/from, colleagues, patients and their representatives in a professional and confidential manner sharing only when it is appropriate to do so.
2. Communicate information to patients using encouragement and reassurance and to overcome barriers to understanding with patients whose understanding is impaired ensuring compliance with sharing of personal information and Data Protection.
3. To report changes accurately and promptly in a patients condition to the care coordinator, using observations on CM2000 or over the telephone with the Service Duty Desk/Management.
4. To be able to maintain records and documentation in accordance with local policies and Trust Guidelines and follow instructions/directions from Service Management Team.
5. To promote positive working relationships within the team, hospital and community colleagues.
6. To take direction from the Service Management Team and implement any proposed patient orientated plans and goals. Report and escalate as appropriate to the office any reluctance if the patient refuses participation.

Clinical Responsibilities
1. To follow a care plan to deliver hands on care to patients within their own home, or the hospital environment. This will include personal care tasks that may involve bodily fluids.
2. To develop an understanding of enabling patients to use functional activities.
3. To demonstrate a holistic understanding of patient and family dynamics, the patients condition and its impact on function particularly in crisis.
4. Provide advice to patients/carers on safe use and care of prescribed equipment and maintain documentation relating to this.
5. To work flexibly across a 7 day week, this will include alternate weekends. The ability to work in accordance with the required shift patterns. (7am 1.30pm or 3.30pm-10pm)
6. To supervise the administration of prescribed medication in accordance with delegated healthcare protocols.
7. To use initiative when responding to changing patient signs (e.g. ceasing treatment if patient unwell) and report back to the office.

Planning and Organisational Skills
1. To show both flexibility and initiative when undertaking daily routines and be appreciative of the needs of all service users and their individual circumstances.
2. To prepare for tasks/environment in line with risk assessment and in order to maximise safety.
3. To participate in required shift patterns.
4. Good timekeeping is essential along with the ability to use specific trust logging in equipment/ devices.
5. To comply with lone working policy for own and others safety.

Service and Personal Development
1. To work with your line manager to implement policies within the team.
2. To participate actively in sharing and receiving in-service training and development to ensure continuous professional development (as appropriate to grade and experience) and compliance with Statutory Legislation.
3. Participates regularly in supervision and identifies and recognises own training needs. Facilitates own learning in these areas.
4. To undertake qualifications as required in line with the Service requirements of the delivery of service. (e.g. Apprenticeship in care / NVQ 2 in care or equivalent as required)
5. To complete the Trusts Care Certificate qualification
6. Recognise and demonstrate to both existing and new staff, clinical and non-clinical duties as appropriate or when asked to do so.
7. Be aware of, implement and adhere to infection control procedures which are appropriate to individual working environments.
8. To educate others informally about the service.

Information and Resource Management
1. Observe a personal duty of care when using equipment and be aware of criteria for provision.
2. To promptly complete data collection as directed by Service Management Team (Head of Service / Line Manager etc).
3. Ability to use the Trust Intranet, e-mail and data bases as well as CM2000 to access and record information.
4. Ensure all patient visits are recorded accurately using CM2000 log in number at every visit.
5. Reporting any additional visits not logged through CM2000 to the office before the end of every shift.

4. To frequently exert moderate physical effort for short periods during the day.
5. To be competent in completing risk assessment for patient, environment and equipment. To incorporate this into daily practice of self and actively encourage others to do the same.
6. Acts as a role model in complying with health and safety policies and demonstrates good understanding of the relationship between posture and safe handling for self and others.
7. To be aware of and dealt appropriately with any exposure to bodily fluids. Frequent exposure to bodily fluids.
8. To risk assess situations and share information with community colleagues as appropriate.
