Specialist Nurse Practitioner Band 6 37.5 HOURS PER WEEK. (MATERNITY LEAVE COVER FOR 8 MONTHS)

We are looking for a nurse with suitable recent anaesthetic or surgical background to join our team. The successful applicant must bequick thinking, dynamic, adaptable, and highly motivated to complete a fast-track induction training within Preop assessment. The successful applicant will be supported with mentorship over an 8 week supernumerary period.
The post involves undertaking detailed assessments of current health status and past medical history from patients with complex medical histories within a busy clinic environment forpatientswho are awaiting procedures from multiple differing surgical specialities.
The ability to converse, question & impart clear instructions pitched at the level of understanding of the patient is essential for this post.
Due to the requirements of the role excellent level of spoken fluency and written communication in English language is essential.
This post is not suitable for newly qualified nurses (minimum 3-year post registration working experience is required)
*Due to the specialist nature of the post, you may be offered a Band 5 position for the first 12 months with a view to upgrade afterwards.

Main duties of the job
- The post holder is responsible for the pre-operative anaesthetic and medical assessment of patients being prepared for a variety of surgical procedures.
- Provide a comprehensive anaesthetic and medical assessment, including baseline outcome measurement, ensuring that all necessary investigations and results are collated prior to the patient's admission.
- Liaise with Anaesthetists/Surgeons regarding potential medical problems and issues regarding surgery. This role will also include appropriate onward referral to primary and secondary care providers to ensure pre-operative health is optimised.
- To liaise closely with clinic administrators with regard to booked admissions, cancellations and DNA's.
- To liaise closely with ward managers, consultant surgeons and anaesthetic staff, nurse specialists, dieticians, therapists, discharge coordinator and MDT with regard to complex admissions and patients with potential discharge problems.

- To effectively manage a caseload of patients/clients
- To actively develop evidence based clinical nursing practice in line with current research and guidelines

- To liaise closely with ward managers, consultant surgeons and anaesthetic staff, nurse specialists, dieticians, therapists, discharge coordinator and MDT with regard to complex admissions and patients with potential discharge problems.

KEY RESPONSIBILITIES:
Clinical
- To provide specialist nursing advice and support to patients, their families and other healthcare professionals from point of decision to carry out surgical intervention through to admission.
- To actively contribute to the reduction of length of stay.
- To carry out detailed preoperative assessment of elective patients, including initiation of discharge planning procedures.
- Assist with the development of assessment and referral protocols and guidelines with medical/anaesthetic/nursing staff.
- Perform and document medical and nursing pre-operative assessment. This includes patient history, health screening and carrying out a medical examination. Give pre-operative advice and support and initiate appropriate referral to other members of the multi-disciplinary team, as part of the patients surgical pathway.
- To carry out/ initiate preoperative investigations/diagnostic tests in line with existing pre-assessment protocols & NICE guidance..
- Exercise clinical judgement and decision making about patients needs in terms of suitability for surgery, consulting Clinicians as necessary.
- To be responsible for following up and reviewing investigation results and alerting appropriate clinicians as necessary.
- Liaise and/or refer to primary or secondary care providers to ensure pre-operative health is optimised.
- Promote lifestyle changes to patients, as indicated, e.g. weight loss, smoking cessation.
- To work autonomously by accepting responsibility for own patient caseload, ensuring all patients have an accurate plan of care, which reflects the assessment undertaken and incorporates the issues and recommendations made ensuring clear documentation in the patients records (hand held and electronic records)
- Liaise with multi-disciplinary teams on the provision of the service to ensure the highest quality of patient care. Provide clinical advice to other multi-professional team members in the area of pre-operative assessment.
- Ensure that evidence based written patient/carer information is compiled, as necessary, and available for patients to ensure they fully understand the proposed surgery and anticipated post-operative recovery.
- Continue the informed consent process by ensuring the patient is aware of the risks and benefits of the procedure.
- Initiate discharge planning and identify any potential problems that may delay discharge prior to admission. Refer to appropriate primary and secondary services, as necessary. Provide patient and carer with realistic estimate length of hospital stay and advice on arranging home circumstances following discharge.
- To support and promote an integrated approach to quality, which will include health promotion and education.
Managerial
- Assist with the management of the pre-assessment workload and clinical area, including recruitment and retention of staff, induction programmes and annual staff appraisals.
- Develop good interpersonal relationships with multidisciplinary staff through effective communication and sound management systems.
- Ensure that complaints are promptly dealt with and reported using agreed procedures.
- Report to the Line Manager any untoward incidents which may have legal implications and/or where staff disciplinary action may result.
- Maintain acceptable levels of stores, equipment and supplies within budgetary restraints, ordering new equipment in consultation with Line Manager.
- Continue to promote pre-assessment as the cornerstone of effective elective and urgent surgical care and throughput.
Education, Training & Development
- To provide clinical expertise and advice to other staff and facilitate training and development to the multidisciplinary team.
- Develop a learning environment for trained and untrained staff within the multidisciplinary team.
- Assist with the development of clinical networking with other centres of excellence and departments actively involved in the enhancement of care for the pre and peri-operative patient.

- Maintain accurate records of patient referrals, treatment and outcomes for purposes of audit and presentation of service outcomes.
