Job summary
We are looking for an enthusiastic, motivated Nurse or Therapist with an understanding of the benefits of early discharge and rehabilitation within a familiar home environment. Someone who can work within a Multi-disciplinary Team, using their initiative to make decisions around complex issues.
The successful candidate will promote and implement the Discharge to Assess principles and pathways. They will identify, assess and co-ordinate services to support the safe discharge and transfer of patients with complex needs, liaising with the Multi-disciplinary Team and partner agencies across acute and community settings.
You will be either be a qualified Registered Nurse, Occupational Therapist or Physiotherapist.
Main duties of the job
- To ensure patient discharge care needs are assessed, planned implemented and evaluated using research evidence as appropriate.
- To provide leadership to facilitate excellent nursing or therapy care in order to promote rehabilitation, patient independence and therefore earlier discharge from hospital.
- To ensure all nursing or therapy procedures are carried out in accordance with Royal Derby Hospital policies taking appropriate action in emergency situations.
- In conjunction with the multidisciplinary team, be responsible for setting, monitoring and maintaining excellent standards of care particularly for patients with multiple needs who require complex packages.
- Maintaining personal contact with patients, relatives and carers being sensitive to their needs for courtesy, dignity and privacy and ensuring a friendly environment at all times.
- Ensuring that accurate and timely records of discharge planning assessments, planning, interventions and contacts are maintained
