Job summary

Are you ready to take the next steps in your career and develop your leadership skills, manage, and lead on quality improvement projects and work together with the MDT to further enhance the service our service provides to patients?
Alongside leadership and development of management skills, with the support of the team and Matron, the post holder will also be responsible for delivering pre and post operative care for patients undergoing life changing surgery whether that be temporary or permanent. The post holder will need to travel to undertake visits to patients in their own homes, on wards and in clinic settings, therefore being a car owner and driver is essential.
As core members of the Colorectal Multi-Disciplinary Team, the Stoma Nurses are committed to working within the framework of the National Cancer Service Guidelines. To initiate contact with patients prior to Stoma surgery in Acute and Primary settings in order that the patients receive support, information and holistic management of care. Thus, ensuring a smooth transition from Acute to Primary Care.
Main duties of the job
To be clinically accountable and responsible for the provision of an efficient Stoma Care service within the community and hospitals of Southern Derbyshire.
To deliver planned holistic support encompassing the physical and psychological aspects of stoma surgery pre and post-operatively, working with approved protocol and guidelines.
To maintain effective communication/liaison with medical and nursing staff, pharmacists and other relevant statutory and voluntary agencies, ensuring a comprehensive quality service is delivered to patients.
To maintain accurate contemporaneous records of all patients to ensure professional/legal obligations are met.
To act as specialist resource for medical nursing staff within both the Community and Acute settings
Initiate design and implement formal education and training programmes for all nursing levels within the Community and Acute sectors.
To raise awareness with regard to Stoma care for medical, nursing and other disciplines as appropriate. On-going follow-up evaluation and monitoring programme.
To work within agreed Clinical Governance framework, in regular audits to continuously improve the quality of service and its effectiveness and future development. Ensuring practice is research based.
