The ARCH Home Visiting Service is a Primary Care Network team covering the geographical area of Amber Valley, Ripley, Crich and Heanor-hence ARCH.

This team is part of the Ageing Well programme They have been providing acute home visits to housebound patients for just over a year. They are a rapidly expanding team having started with just one GP and now are a multi-disciplinary team of 19 (and counting). In addition to home visits, they also provide support to care homes and undertake proactive care such as advanced care planning.

Main duties of the job

This is a new and exciting post that aims to cross the divide between primary and secondary care. It will provide excellent training in both acute frailty and community geriatrics alongside an opportunity to develop leadership and management skills.

Essential

  • MBBS or equivalent medical qualification
  • Advanced Life Support certification
  • Part 1 MRCP

Desirable

  • Part 2 written and PACES MRCP examinations

  • Full GMC Registration with a licence to practice at time of application
  • A minimum 4 years post qualification medical experience in the UK or abroad, including experience in Acute Medicine/ Medical specialities
