Job summary
An exciting opportunity to work with Northumbria Healthcare Public Health team's tobacco dependency treatment service. This team is key to delivering the ambitions of the NHS Long Term plan to provide treatment for tobacco dependency for all patients admitted to hospital, making a significant contribution to public health and reducing health inequalities.
This post holder will specifically support work in our Emergency Department and our in patient wards at NSECH. Travel to other hospital sites may also be required.
We welcome applications from those with practical experience in a health and care setting.
Main duties of the job
This post will involve working directly with patients to assess their needs related to tobacco dependency and to work with them to identify treatment and support to enable them make a successful quit attempt. Support may be face to face, remote consultation or via telephone.
Other duties will include supporting the wider public health team in developing and delivering training packages on tobacco dependency and a range of public health issues.
To support the day to day running of the tobacco dependency service to ensure safe, well organised and evidenced based care is provided to all that access the service. Provide support in a range of settings as required.
To support the delivery of the service key performance and reporting indicators for tobacco dependency.
Organise own workload and work unsupervised in a busy team. Maintain a high standard of work and prioritise own workload according to the needs of the service, individual needs including equality, diversity and social inclusion in all aspects of work.
Take referrals, including self-referrals into the service and working with patients to identify the most appropriate tobacco dependency treatment pathway.
Identifying and engaging with individuals and groups from target areas.
Supporting individuals to make informed choices about their health and wellbeing and supporting them in initiating and sustaining appropriate behavioural changes to improve their health.
