Job summary

An exciting opportunity has arisen for an enthusiastic individual to join the Community Nursing Team as a Nursing Assistant within the Blyth District Nursing team. The Healthcare Assistant within a community setting works as a member of a wider community nursing team to delivery clinical care to patients including the performance of diagnostic tests and the delivery of personal and delegated nursing care to individuals with chronic health problems. Although the post holder receives support and supervision from registered nurses she / her is expected to work unsupervised in the patient's own home, the surgery and in residential home environments. The health care assistant will have delegated responsibility to order and rotate stock to support the team. The post holder will also undertake administrative tasks allocated by the team lead for example, assembling patient case notes, room bookings to facilitate the arrangement of team meetings and sickness absence meetings.

Main duties of the job

  • The performance of personal care tasks and clinical interventions delegated by registered nurses including simple dressings, enemata, catheter care, eye drops etc.
  • The delivery of personal care and support to patients with a terminal illness and their families including the provision of emotional support.
  • The collection of equipment and the safe use and decontamination of such equipment between each use.
  • Good communication skills with patients, relatives and colleagues. Communication with patients, relatives, colleagues and other agencies to ensure the continuity of patient care and onward referral where necessary.

  • The checking of equipment, stock levels and expiry dates re-stocking and re-ordering supplies as necessary.
  • IT skills especially in relation to the use of System One and General Practice clinical systems
