Job summary
We are looking for enthusiastic, dedicated nurses to join our teams in the North Tyneside and North Northumberlandareas and work with us to deliver an evidence based, clinically effective service for our patients in their own homes.
The post holder will be part of a wider Community Response Team but link in closely with the District Nursing Teams in the North Tyneside and north and west of Northumberland
You will assess ,plan, and evaluate clients careincluding families and carers were appropriate
The service is part of an integrated community pathway for patients on our Virtual Ward also offering a crisis response and supporting with unplanned care for patients in their own homes and in our rehabilitation units . The service will provide an integrated approach to care of patients in the community by using a holistic approach and providing a multidisciplinary assessment which will aim to ensure patients remain at home with appropriate health and social services support.

Main duties of the job
The Community Staff Nurse will work in partnership with patients and carers in the community, providing health education and nursing care within the Scope of Professional Practice. The post holder will be an integral member of the Community Response Services working with colleagues to deliver clinical care to patients on our Virtual wards and those in thier own homes undergoing a health crisis needing and urgent response
Although the post holder has access to advice he / she is expected to work largely unsupervised. In addition, the post-holder is expected to supervise the work of others including Health Care Assistants, Associate Therapy Practitioners and students on placement.

- The performance of diagnostic tests such as bladder scanning ,Ankle Brachial Pressure Index using Doppler and the interpretation of the results to decide further courses of action.
- Performance of procedures which require fine dexterity and skill such as venepuncture, cannulation, drug administration by injection and the removal of sutures.
- Interpretation of results and the communication of these to patients e.g. blood glucose estimation. Explaining aspects of care to patients and carers allowing the opportunity to ask questions and raise concerns
- Maintenance and promotion of high standards of professional practice including the sharing of approaches and experience with others
- Maintain a duty of care to patients, carers and colleagues through the exercise of personal and professional accountability

- Additional clinical qualifications or equivalent derived through experience at diploma / degree level if experienced
- Newly qualified staff will be required to undertake the Trust preceptorship programme within the first year in post.
- Mentorship Qualification or relevant equivalent qualification (essential requirement for post, expectation this will be completed within an 18-month period)
