Job summary

This is a fantastic opportunity to join an innovative and supportive Nutrition and Dietetics team at an outstanding trust.

We are looking for a Health and Fitness Facilitator with a passion to make a difference to patients with obesity.We are excited to be able to offer a band 4 post to be a key member of our Tier 3 specialist adult weight management (AWM) service, and work collaboratively with dietetic staff and the wider team to provide a high quality service for the population of Northumberland.

The post holder will provide health and fitness activities as part of the pathway for those referred and will include assessment of physical activity level, active conversations that lead to physical activity plans and individually tailored exercise interventions to improve health and fitness.

The post holder will work as part of a multi disciplinary team (MDT), including specialist dietetics and psychology, and will require excellent communication skills to promote and encourage healthier and more active lifestyles.

Main duties of the job

The main responsibilities of the role will be to work alongside our specialist teams to review current service provision, work with partners to develop the service moving forward and to increase patient satisfaction and successful completion of the Tier 3 AWM program.

The post holder will support the delivery of the physical activity elements of the service by encouraging those referred to increase physical activity through motivational support, goal setting, action planning and follow up support.

You will be an integral member of a multi-disciplinary team working to support the safe and effective delivery of the Tier 3 AWM program in Northumberland.

The team includes specialist dietetics and psychology and is building on the current model successfully delivered in the North Tyneside region. There is some service development to provide a blended delivery approach, with both face to face and virtual options, to all areas of Northumberland.
