Job summary
We have a great opportunity for a pharmacist wishing to develop their career as part of our progressive team, with a national reputation for innovation and development. We have the highest proportion of pharmacist prescribers in the country, using their skills for patients on acute hospital wards, in specialist clinics, GP practices and a range of community settings. You may already be a senior clinical pharmacist, awaiting a new challenge, or an experienced pharmacist ready to take the next step in your career.
We are delivering new ways of providing direct patient care in acute and community settings, including providing care to our frail/high risk patients living in their own homes or care homes. We are also increasing our service within primary medical care practices, enabling delivery of care to patients with long-term conditions as part of a multidisciplinary approach.
Main duties of the job
As a senior clinical pharmacist, you will be a motivated individual who will assist in shaping the service and the clinical contribution we provide. Within your own area of responsibility you will:
Provide medicines optimisation for people with acute and long-term conditions;
Provide leadership in the delivery of highly specialised settings; and
Manage the care of specific patients.

We manage three major locality hospitals at North Tyneside, Wansbeck and Hexham, plus a number of smaller community hospitals and clinics from Tynemouth to Berwick on Tweed, in addition to our state-of-the-art Northumbria Specialist Emergency Care Hospital, the first of its kind in England. We also care for people in their homes and provide services from facilities in local communities such as health centres. We give people greater choice and control over their care to help them to live independently at home and avoid hospital admission where appropriate.
