Job summary
An exiting opportunity has arisen to join North Tyneside Community Nursing Service, as a Clinical Operational Lead/ Neighbourhood Matron working across a cluster of Practices within a defined geographical area. The CommunityClinical Neighbourhood Matron will have overall responsibility for identification of vulnerable patients who are at risk and also those frequently admitted to hospital by proactive case management working with Specialist Nurses, Medical colleagues, Palliative Care, IntermediateCare, Social Care and AHP's. The aim is to reduce admissions and improve quality of life. In addition, the post holder will lead and be responsible for District Nursing teams within the neighbourhood by developing new effective ways of working.
The Clinical Neighbourhood Matron role models compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the Trust demonstrating the creation of a collective leadership culture within the trust. This means demonstrating a consistent leadership style, engaging, enabling and empowering others, promotingownership of learning and quality improvement.
Main duties of the job
The post holder will be responsible for operational management and clinical leadership across a primary care network working directly to the Senior Manager, maintaining a high standard of care delivery to patients, clients and relatives.
The post holder will be required to represent the Community Nursing service, working with other health and social care providers to shapean integrated whole system approach to service delivery across the primary care network.
The post holder will work towards advanced professional practice including advanced clinical skills, medicines management, leadership, supporting self-care management and forge interagency and partnership working to improve outcomes for patients with co-existing long term conditions. In addition, the post holder will work proactively to reduce unplanned hospitaladmissions and facilitate earlier discharge from hospital.
