Job summary

The post holder will work closely with medical, nursing, therapy, and clinical support services to ensure that patients receive appropriate interventions, investigations, and treatment with minimal delay. The post holder will be part of a Rota to cover the service which will run 8am-8pm, 7 days a week.

Responsible for the holistic assessment, planning, implementation and evaluation of evidence-based nursing care for patients referred to hospital at home service.

Providing high-quality care for patients including co-ordination of the multidisciplinary team and allocating visits as appropriate.

Participate in effective clinical leadership and management of team members through clinical supervision.

Actively contribute to the development and improvement of services and the quality of care through reflective practice, clinical supervision and audit.

Act as a role model, mentoring, teaching, assessing and supporting students and learners on placement and participate in the education, development and mentorship of other staff.

Main duties of the job

The role responsibilities will include:

Care Delivery

- Demonstrate high level of judgmental/analytical skills when interpreting and performing patient assessments and care planning via the telephone and face to face.

- Maintain accurate and up to date patient care records, maintaining patient confidentiality, and statistical records as required by the service.

- Work with other professionals to promote multidisciplinary / multiagency working.

- Undertake clinical skills e.g Phlebotomy, ECG's, Physical Observations and monitoring.

- To attend and contribute to virtual ward MDTs

- Triage of patients referred to the virtual ward hub highlighting clinical need, initiating treatment, and making referrals to senior medical or specialty colleagues as required.

- Following up referrals and supporting the discharge of patients with the digital provider supporting the Virtual Ward programme.

- Liaising with the relevant senior medical, nursing, allied health professionals (AHPs) and pharmacy teams and stakeholders across the Integrated Care System (ICS) e.g., GPs, community / primary care network pharmacists, district nurses & social workers to promote safe, evidence-based delivery of care within the medical division virtual ward pathways.

We have a dedicated team working in our virtual ward medical hub in a multi-professional team. We have an ambitious aim for our NHS@Home Virtual Ward pathways to be truly integrated between primary, community and secondary care and to offer a high quality, alternative to acute bedded care for the people of Gloucestershire. These roles offer an exciting new opportunity for skilled nurses and AHPs to develop the service with our dedicated medical and community teams and to gain experience working at the interface between community and secondary care for acutely unwell patients.

The service will aim to manage patients safely at home with a range of medical conditions who would otherwise be in hospital such as respiratory, frailty, heart failure - with the support of specialty teams and our digital partner we believe that this is an exciting time to be joining this new team.
