Job summary

The Specialist Palliative Care Team provide complex symptom control interventions as well as managing complex needs to patients across the Trust, as well as providing education and support to fulfil the Trust's end of life improvement plan. We are looking for a registered nurses with experience in palliative care to join our team to cover a period of maternity leave.

You would be expected to work as a clinical nurse specialist and with support of the team, assess and manage patients with complex palliative care needs.

We would consider a combination of working patterns for the right candidate

Main duties of the job

We are looking for an euthastic and motivated people who have insight into the role. Knowledge and skills around symptom management as well as managing complex psychosocial issues and an ability to manage sensitive and honest conversations. Working with a supportive team, we work 9-5, Monday - Friday with rotation onto the oncall service rota.

To work effectively as an autonomous practitioner within the countywide multidisciplinary palliative care team promoting and maintaining quality of life for patients with cancer or other life threatening illnesses requiring specialist palliative care and into bereavement for relatives and carers

To provide specialist clinical leadership in the assessment of complex symptom and psychological problems, provide support, advice and leadership to health care professionals in line with national and local guidelines ensuring a cohesive, holistic and evidence based approach to the management of palliative patients.

To empower and equip health care professionals with skills and knowledge to deal with a variety of less complex problems through education and being an available resource
