Trainee/ Qualified Advanced Clinical Practitioner in SDEC/ AIM, Band 7 or 8a

We are seeking motivated people who are seeking to be trainee or Advanced Clinical Practitioner in Same Day Emergency Care with some Acute Medicine rotation.

We have seen the growth of Same Day Emergency Care expand and flourish in the past 12 months.

We have two SDECs located at Gloucester and Cheltenham. Cheltenham offers a 5-day service run by the ACP team and supported with clinical oversight of the acute medical consultant.

Gloucester offers a 7 days service supported by ACPs, and is run by a consultant led service.

Our ACP team in SDEC have presented at Health Education England, and offer a multi-disciplinary team with a wealth of experience to support trainees and have been published in the international journal of Advanced Practice. We have successfully supported ACPs through the apprentice and e-portfolio routes to advanced practice. As we expand our Acute Medicine portfolio, we are keen to see Advanced Practice diversify to acute medicine bringing in a wider portfolio and brining skilled team members across the division.

Main duties of the job

The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.

The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Job responsibilities

Job Purpose

- To act as an autonomous Advanced Clinical Practitioner (ACP) providing high quality, individualised clinical management for patients using advanced clinical assessment and diagnostic reasoning skills to determine and initiate changes to treatment as necessary within Same Day Emergency Care (SDEC)

- To order and interpret radiological imaging, using findings to initiate treatment

- Provide advice, support and education to medical, nursing and AHP staff

- Prescribe medication appropriately for specific groups of patients, following Trust guidance where appropriate

- Work as part of an advanced practitioner led SDEC service, as well as working as part of the multidisciplinary team

- Develop and deliver educational programmes for staff locally and work with advanced practitioners nationally to develop practice

- Act as clinical role model / educator for developing and delivering care that is responsive to changing requirements

- To constructively challenge others and demonstrate receptiveness to challenge

- Take active part in Clinical Governance, research and development initiatives

- Facilitate research-based practice and assist in disseminating research findings and influencing change

- To assist with audit and research projects and participating in up-dating and implementing departmental policies, protocols and guidelines, in line with national guidance and contemporary evidence

- It is anticipated that 80% of the time will be spent working clinically in the clinical area and 20% will be allocated to non-clinical duties.

Knowledge, skills and Experience Required

Clinical

- Registered practitioner (Nursing or Allied Health Care Professional) with extensive years experience relevant to role

- Masters in Advanced Clinical Practice or Masters level award that covers all 4 pillars of the Advanced Clinical Practice (ACP) capabilities (essential to include physical assessment and physiological reasoning qualifications) or if in the trainee position you will be offered an apprentice Advanced Clinical Practitioner with a Higher Education Institute in the local region.

- Non-medical prescribing essential if applying for a qualified position, with evidence of updating knowledge appropriate to patient groups prescribing for.

- Enhanced aptitude required to initiate, evaluate and modify a range of interventions

- Develop scope of advanced clinical skills and practice to meet changing needs of service

Education

- Recognised teaching qualification or relevant experience

- Demonstrate an understanding of research and evidence based practice

- Enhanced IT skills working knowledge of software packages such as Word, Excel and Outlook alongside PACS, ICE etcResearch

- Demonstrable ability to participate in research and/or audit activity, adhering to good research practice guidance so that evidence-based strategies are developed

Leadership

- Effective communication skills, able to communicate highly complex and sensitive information
