Homeward Assessment Team Frailty Occupational Therapist/ Physiotherapist

An exciting opportunity has arisen at GHNHSFT to join the Homeward Assessment Team as Frailty Occupational Therapist/ Physiotherapists . Within our medical division we are looking for experienced and enthusiastic individuals to join the Homeward Assessment Team (HAT). HAT are a multidisciplinary team which includes the Frailty Assessment Unit who recently merged with GHC IAT team. The Frailty Assessment Unit and Care of the Elderly department are also currently undergoing development of the service in line with the Fit for the Future work. This is a very exciting time to join our team as we build on our existing service in order to support patients to remain in their own home and avoid hospital admissions.

We are looking for enthusiastic individuals to provide therapy support to the Frailty Assessment Unit and Emergency Department. The successful candidate would need to work in a fast paced environment, taking a positive risk taking approach. You will need excellent communication skills, leadership and commitment to high quality patient care alongside flexibility and resistance to meet the challenges of this role.

Main duties of the job

We are looking for individuals who have proven NHS experience and thrive working within a multidisciplinary team. We are looking for individuals who have innovative ideas to promote admission avoidance using a positive risk taking approach alongside clinical reasoning. This role is fast paced and you would be a key part of the team, providing assessment and planning for discharge. You will have a knowledge of community services to signpost/refer to.

You will be supporting the day to day delivery of the HAT assessment working towards achieving a positive outcome for patients following the most appropriately indicated pathway, assisting in admission avoidance where appropriate. The successful candidates would need to demonstrate a flexible approach as the demands of the role may change depending on the demands of the service.

We will offer you the opportunity to work in a supportive multidisciplinary team, with access to training, regular supervision, annual appraisal.

2. JOB PURPOSE

- To support and participate in the assessment, planning, implementation and evaluation of high-quality holistic patient care, within the Frailty Assessment Unit and Emergency Department working as part of the Homeward Assessment Team.

- To provide a local link to the wider networks for older peoples services through knowledge and skills pertinent to the care of older people.

- To use recognised models of practice and evidence-based interventions and outcome measures to underpin all practice.

- To assess and treat own caseload of patients adopting a person-centred approach.

- Promoting the role of therapy with the wider Homeward Assessment Team.

- Participate in research/project work/service development as opportunities arise.

3. CORE RESPONSIBILITIES

- To be responsible for carrying out evidence-based assessments, intervention and evaluation of mobility and occupational performance needs of patients. You will assess, provide advice, appropriate equipment, and refer on to community teams and services.

- To be competent in risk assessments and managing risk autonomously.

- To provide advice and guidance regarding mobility and function of patients to the MDT.

- To manage a diverse caseload of patients with varying levels of frailty including conditions such as orthopaedic, medical, neurological and palliative with a view to avoid hospital admission where possible.

- To understand the nature, aetiology and expected progression of a wide variety of diseases and to signpost to community teams as appropriate.

- To have an excellent knowledge of resources available outside of the Acute Trust.
