Job responsibilities
Job Purpose
- Ensure the delivery of high nursing care by ensuring appropriate systems and processes are in place to provide the continuity of safe and effective care at all times.
- Communicate effectively with patients, carers, relatives and all members of the multidisciplinary team.
- Effective management of own time to manage caseload of patients within a defined scope of practice.
- Work within professional accountability and standards, ensuring that professional practices are in line with the requirements of the Nursing and Midwifery Council (NMC) and Trust policy.
- Take on responsibility for personal development and support other members of the team.
-Provide assurance that the clinical governance procedures and protocols are embedded and adhered to in respect to risk, safety, and quality improvement.
Patient Care
- Demonstrate and promote a patient and family centred style of clinical practice.
- Demonstrate ability to assess patient condition and implement appropriate care, within the multidisciplinary team.
- To communicate effectively, especially where patient ability to do so is compromised.
- Ability to disseminate contentious and delicate information appropriately.
- To deliver safe care within a challenging and changing clinical environment, using equipment and resources safely and as appropriate.
- To administer medication and treatment within Procedure for the Ordering, Prescribing and Administration of Medication Guidelines.
-To deliver and receive information which may cause emotional distress to all parties.
-Participate in mentorship and educational programmes for students and other members of the team.
- To adhere to all Trust policies and procedures and professional code of conduct.
- Regularly responsible for the supervision and mentorship for unqualified staff.
- To input and retrieve patient and staff information from computer-based systems.
- Maintain accurate and contemporaneous records.
- Undertake assessments, planning, perform investigations within scope of knowledge: order tests, interpret results and act on findings. This necessitates high levels of autonomy and advocacy
- To be responsible for the organisation of own caseload and appropriately delegate to support workers or other staff members in order to ensure effective time management. To assist the team in the day to day running of the clinical area.
- Respond to emergency situations with composure, maintaining patient safety and seeking immediate support, supporting the patient, family and other staff as required.
- To communicate effectively with patients and carers potentially complex, sensitive or distressing information. Patients may have communication, perceptual or cognitive/understanding difficulties.
- To maintain clear, accurate and concise patient records in line with departmental and professional standards and include relevant outcome measures. This will include writing patient reports for e.g., medical practitioners and solicitors requests.
- Maintain patient dignity and confidentiality at all times.
Professional Practice
- Ensure clinical practice is evidence based or peer reviewed best practice
- Develop and maintain clinical skills and knowledge necessary to deliver and evidence-based service.
- Be competent in the use of all relevant Trust IT systems including the use of electronic patient record, Trak care, email, and PACS.
- Maintain regular use of emails to keep abreast of information disseminated in the Trust
- Adhere to the NMC code of practice at all times and operate within the NMC scope of professional practice.
- Demonstrate an awareness and understanding of Accountability, Responsibility and Duty of Care and work towards the provision of support and education for less experienced and non-registered staff on the implications for practice.
- Report near misses and untoward incidents, complaints, clinical emergencies, injury or medicine administration errors as detailed in the departmental and Trust protocols.
- Conduct oneself in a professional manner and be a role model to less experiences/ non registered and new staff at all times
- Ensure a smart, professional image that enhances the public perception of the Trust and the professionalism of its staff is portrayed.
Most Challenging part of the job
- Comply with requirements to register with the NMC/GDC and identify own learning needs and ensuring own professional development is maintained by keeping up to date with practice developments.
- Regular interruptions from enquiries and or changing patient needs
- Possible activity changes requested by the multidisciplinary team due to changing priorities
- Exposure to distressing or emotional circumstances, upholding a caring and professional approach during distressing and difficult situations.
- Caring for patients of all ages with facial disfigurement and impaired communication
- Maintain high personal operating standards whilst guiding and assisting less experienced medical staff
- Prioritise tasks and clinical needs to accommodate short notice changes to clinic schedules
- Dealing with complaints and unpleasant remarks of aggrieved and frustrated patients
Physical Effort and Working Conditions
- Will be required to work within recognised Manual Handling guidelines.
- May be expected to frequently manoeuvre patients/objects over 15kg using appropriate aids for short periods of time.
- Will have frequent exposure to unpleasant working conditions and will be required to follow Policies and Procedures pertaining to bodily fluids, infection control and COSHH regulations.
- May be required to work in confined spaces.
- Ability to work where the work pattern may be unpredictable, but there is also a requirement for concentration e.g., drug administration.
