An exciting opportunity has arisen for enthusiastic Staff Nurses and Healthcare assistants to join our dynamic Frailty Team. Frailty is a rapidly growing specialism that aims to reduce hospital admissions for frail and elderly patients in an assessment type environment.

This is a developing service and we are searching for motivated and enthusiastic individuals. Ideally you would have a background in Frailty or Elderly Care however we welcome applications from any interested candidates.

Main duties of the job

We are looking for Registered Nurses who deliver compassionate, professional, skilled and effective person centred, evidence based nursing care by working within a multidisciplinary team, developing collaborative, needs based care plans through assessment, planning, implementation and evaluation.

We work closely as an MDT with Therapists, Doctors, Nursing staff and Pharmacy. We try to involve patients and their relatives in their care as much as possible.
