An exciting opportunity has arisen at GHNHSFT. Within our medical division we are looking for an experienced and enthusiastic leader to join our newly integrated Homeward Assessment Team (HAT).
Having recently merged both the Frailty Assessment Service (GHT) and Integrated Assessment Team (GHC) the HAT team are a multidisciplinary team who offer a combined assessment of frail and non-frail pathway through emergency/assessment areas.
As we develop this service we are looking for an enthusiastic individual to lead the overarching integration through the next steps to work with the clinical and operational leads to develop a resilient and integrated team.
The successful candidate will support development of internal and external processes and pathways in line with the strategic direction.
The vision of the integrated team is one team with a focus on improving patient experience through admission avoidance with high quality, early, specialist assessment.
Main duties of the job
The Integration Lead for HAT will provide leadership across two multidisciplinary teams that are in the process of integration. The teams are based within emergency and short stay departments at GHNHSFT.
The primary purpose is around leadership and accountability surrounding Integration and promoting a one team direction.
You would be responsible for supporting the day to day delivery of the organisations contractual obligations in respect of the HAT service specification.
Critically review the daily integration processes to identify areas that can develop new and innovative ways of working.
Be responsible for delivering the outputs of the integration programme task and finish groups.
Be responsible for team building, conflict resolution and organisational development activities.
Be responsible for creating an active learning and quality improvement environment.
