We are recruiting for two Deputy Leads for the Onward Care Team within Gloucestershire Hospitals NHS Foundation Trust. This is an exciting time to join the team as our One Gloucestershire ICS implements new ways of working that help us to embed #HomeFirst as our default discharge plan. We require dynamic, driven, positive people who embrace change and indeed, identify the change needed, to help us support even more of our patients to return home when they are ready to leave hospital.
Main duties of the job
- A minimum of 5 years experience within Health and Social Care with recent and relevant understanding of onward care planning within One Gloucestershire.
- Recognised evidence of leadership experience and working towards a necessary qualification in this area.
- Experienced clinician with an understanding of the impact that multiple comorbidities, hospital admission, deconditioning and risk can have on onward care planning.
You will form part of a strong, proactive, dynamic senior triad for the Onward Care Team that drives its further development.

Job responsibilities
- To provide daily operational support to the OCT in their role to facilitate a patient-focused approach to onward care planning that supports patient choice, reduces unnecessarily prolonged hospital admission and supports the Trust and wider Health and Social Care community in implementing a Home First approach to onward care.
- To support the development of robust education and training infrastructures within OCT that are founded in evidence-based practice and in line with national guidelines.
- To expand on this education further by creating a programme of training and teaching that supports ward MDTs across all Divisions to embed discharge planning from the point of admission using tools that include for example; Criteria-Led Discharge, Red2Green and Right to Reside criteria.
- To develop a cross-organisational process that enables vigorous identification of patients with likely complex needs, timely action to support minimised length of stay and comprehensive plans to reduce unnecessary readmission in the absence of acute medical/surgical concerns.
- To be an expert clinician for the OCT and ward teams by providing information, advice and support to health care professionals, patients, relatives and others involved in patient care.
To undertake audit and/or service improvement projects that aim to improve the hospital journey and onward care planning for patients. To use Datix, complaints or concerns in relation to discharge planning, to inform these projects and share learning, monitor for trends and take the necessary action.
