Job summary
An exciting opportunity has arisen for a Band 4 Physiotherapy Associate Practitioner to join the Community Response and Rehabilitation Team, Community Directorate NUTH. The successful applicant will work alongside a community based multi disciplinary team, working across a variety of community settings. The main focus of the role will be supporting physiotherapists within the Community Response and Rehabilitation Team by carrying out patient assessments and interventions within the patient's own home. Specific tasks could include walking aid prescription, exercise support and progression and equipment provision under the supervision of physiotherapists. The post holder will demonstrate an ability to work independently when required and prioritise their own caseload.
The successful applicant will have had relevant experience either in the community or as a therapy assistant or technical instructor within an appropriate field and bring skills including, effective team working, highly developed communication, IT and moving and handling skills. Attributes will include being flexible, honest, reliable and willing to learn.

Main duties of the job
- Accept a caseload of patients under the guidance of qualified Allied Health Professionals in the team
- Undertake non-complex assessments and treatment plans including providing rehabilitation in the patient's home under the guidance of qualified Physiotherapists / other Allied Health Professionals in the team
- Record outcome measures
- Joint working alongside Physiotherapists and the wider multidisciplinary team
- Be flexible and adaptable to respond and prioritise to the needs of the team caseload
- Work closely with Band 3 Support Workers and qualified Allied Health Professionals within a short term rehabilitation-focused care support service

- To undertake specific skilled support work to aid the Physiotherapy team in the efficient and effective delivery of Physical Rehabilitation to patients within speciality area within the framework of clinical governance
- To accept a patient caseload referred by a physiotherapist according to specific criteria. To treat patients according to set protocols, monitoring patient progress, modifying and progressing patient treatment programmes as required, with referral back to physiotherapists as/when appropriate
- To carry out basic assessment and treatment of non-complex patients as delegated by the physiotherapist (to include basic chest physiotherapy and mobility protocols)
- To provide and deliver individualised exercise programmes
- To accurately record rehabilitation progress as per Trust documentation / database including physiotherapeutic outcome measures
- To undertake a clinical caseload within speciality area , without supervision for at least 50% of the time within protocols and guidelines set by the designated physiotherapist
- To coordinate, supervise and teach junior therapy assistants
