Job summary
We are looking for a highly motivated Lung Health Check Screening Review Meeting Co-coordinator to work within the Radiology Department. Working with the Targeted Lung Health Check (TLHC) Leads, the post holder will co-ordinate and participate in weekly TLHC meetings ensuring all patients requiring discussion are listed for discussion in a timely fashion.

Main duties of the job
- The post holder will also be responsible for coordinating any necessary appointments/investigations, liaising with multi-disciplinary teams, GP's, patients and their carers in order to facilitate individual patient journeys through cancer pathways to reduce avoidable delays in diagnosis and treatment.
- The successful candidate will have excellent communication and interpersonal skills with the ability to adapt their approach in order to manage sensitive and contentious issues, dealing with patients and staff. Working closely with multi-disciplinary teams, they will possess excellent organisational and time management skills with the ability to prioritise tasks and ensure consistent provision of service. The post holder will be required to work cross-site at both the Freeman Hospital and RVI.

- Ensure all patients requiring discussion are listed for discussion in a timely fashion
- Request all relevant previous imaging and ensure all imaging listed for review
- Document outcomes of SRM
- Ensure follow-up imaging re-listed for discussion and management decisions
- Coordinate any necessary appointments/investigations liaising with multidisciplinary teams, GPs, Patients and their carers in order to facilitate individual patient journeys through cancer pathways to reduce avoidable delays in diagnosis and treatment.
- Prepare reports and spreadsheets for LHC audit
- Facilitate communication of results to participants by letter and telephone
- Handover to other teams to facilitate safe and effective transition of care between services in order to provide seamless support for patients.
- Coach patients, their families and carers to ensure they are informed, available and prepared for investigations.
- To perform regular audit of specialist systems to ensure patient reviews are performed at correct interval periods and all follow up actions are completed to agreed schedule.
