Job summary
We are looking to recruit a Physician Associate to support a new and innovative COPD virtual ward at Newcastle upon Tyne Hospitals NHS Foundation Trust. The successful candidate will be an integral part of the multidisciplinary team helping to deliver high quality patient centered care.
The successful candidate will be at the forefront of this new workstream alongside the multidisciplinary COPD team to develop new pathways of care which facilitate early supported discharge for patients through remote monitoring and support.

Main duties of the job
- Diagnosis, risk assessment and treatment for patients with COPD
- Assessment for early discharge and/ or monitoring patients in the community
- Develop and deliver appropriate treatment and management plans
- Maintain timely and appropriate documentation
- Be a key part of the multi-disciplinary team
- Contribute to the development of efficient pathways of care
- Develop clinical guidelines
