Job summary

We have an exciting Band 6 opportunity for a suitably experienced nurse to join our Palliative Care Service at Newcastle Hospitals within Newcastle community as a Palliative and End of Life Care Educational Facilitator. This role involves supporting the Newcastle upon Tyne Care Homes with education and training in Palliative and End of Life Care. You will work alongside the Specialist Care Home Support Team and the Community Palliative Care Team. This is predominately a non-clinical role.

The successful applicant will be able to demonstrate knowledge/skills in palliative and end of life care. The successful candidate must have excellent communication skills and will possess the confidence to educate and engage with all staff groups.

We are looking for someone who is dynamic and driven, able to work autonomously and within a team. Current driving licence and use of a vehicle.

Main duties of the job

To work alongside the Specialist Care Home Support Team and the Community Palliative Care Team to create, develop and deliver Palliative and End of Life Care educational programmes to care home staff. You will have excellent communication, engaging and inter-personal skills. You will be expected to educate and support care home staff and the Specialist Care Home Support Team with advance care planning for care home patients and empower care home staff to continue to care for patients through to end-of-life.
