An exciting opportunity has opened for a dedicated Pharmacy Technician to join our Crisis Resolution Home Treatment (CRHT) team, part of the Urgent and Emergency Care (UEC) pathway within Leicestershire Partnership NHS Trusts Mental Health Services.
Based at the Bradgate Unit/out in the community as required, your role will be to maintain a safe, efficient, and professional medicines management service responsive to the needs of its service users, in accordance with current legislation and guidance. The CRHT team at the Bradgate Unit has a multi-disciplinary approach to patient management. Supported by the Lead Pharmacist, you will demonstrate and develop your professional accountability, innovative thinking and contribute to the clear vision for the CRHT team.
This role offers a Monday - Friday working pattern, with occasional weekend and bank holiday cover on a rotational basis to meet service needs and were happy to consider flexible working patterns that align with service needs.

Main duties of the job
Identify new patients accepted by CRHT team, contact them, GPs and/or other healthcare providers to collate an accurate medication history and documenting this in the patients record. If discrepancies are identified, these should be dealt with or escalated appropriately.
Identify potential pharmaceutical issues that may arise during a patient's care, discharge planning and work with multi-disciplinary team to resolve these.
Work as part of the MDT and be the point of contact for staff when managing medication needs. Liaise with community pharmacies and GPs regarding CRHT electronic prescription issues.
Manage the notification of medication supply issues within CRHT. Escalate complex clinical queries to the pharmacist.
