Main duties of the job
We are looking to recruit an experienced and motivated Band 6 District Nurse to join our community nursing team. Thisrole offers an exciting opportunity to deliver high quality, person-centred care to patients within their own homes and community settings.
The post holder will work autonomously within their scope of practice, assessing, planning, implementing, and evaluating care for patients with complex physical, social, and environmental needs. You will support effective triage and caseload coordination, working closely with the multidisciplinary team and partner agencies to ensure timely, integrated care and to promote independence and self management.
You will provide direct patient care, contribute to specialist nursing interventions, and act as a clinical role model within the team. As a Band 6 practitioner, you will provide clinical supervision, guidance, and day-to-day support to registered and unregistered staff and students, supporting the maintenance of high standards of practice.
The role includes supporting the day-to-day operational running of the district nursing team and deputising for senior colleagues when required. You will contribute to service improvement, quality initiatives, and the efficient use of resources in line with Trust policies, professional standards, and local and national priorities.
