Job summary

An exciting opportunity has arisen for enthusiastic Nurse Associates to join our Community Nursing Teams. This post applies to Registered Nurse Associates or those soon to qualify. If you are passionate about providing quality, compassionate care to people in their own homes and you can drive and have access to a car that you can use for work, this is the job for you! You will be part of a compassionate, forward thinking team working in one of the community nursing hubs. You will work closely with a team of registered community nurses, led by a District Nurse to deliver a wide range of care activities as detailed below. Core service hours are between 8am and 6pm and you will be required to work weekends on a rota basis. As a flexible employer, we have a variety of shifts covering these hours. Main duties of the job

Providing clinical care to patients this includes, but is not limited to: management of wounds and leg ulcers, administration of medicines and catheter care. Providing personal care for patients. Working with the wider multidisciplinary team. Supporting patients to remain at home and avoid hospital admission. Promoting self-care and independence. To recognise and report changes in the patients condition, and any issues in respect of their health and management of care to the registered practitioner.
