Job summary
As a registered nurse, the post holder will work within an integrated neighbourhood team contributing to and supporting the delivery of the Healthy Child Programme 0-5 services.
Working within Healthy Together, the post holder will contribute to improving access, identifying health needs, improving outcomes and reducing health inequalities by providing a comprehensive neighbourhood based service to meet the needs of local children and their families.
Through a whole family approach, the focus will be on prevention of ill health, promotion of positive health and well-being and working collaboratively with the wider community.
As a member of the Healthy Together neighbourhood team the post holder will be responsible for taking an active role with the 0-5 service in the delivery of integrated health services. Training and the competency framework will support all areas of work.

Main duties of the job
- Delivery of effective health care within the organisation.
- Support the delivery of the Healthy Child Programme using evidence based practice to actively promote the health of children, young people and families.
- Develop, deliver and engage in health promotion programmes that aim to prevent ill health, support early identification of health needs and work with individuals, education and communities to reduce health inequalities.
- Carry out universal and targeted 6-8 week, 3-4 month, 1 year and 2 year development reviews.
- Deliver additional support packages of care (infant feeding support, sleep/behaviour support).
- Undertake comprehensive health assessments for identified children and young people.
- Contribute to the triage of A&E attendances and hospital discharges within agreed Trust guidelines, following up priority and potentially vulnerable children, young people and families.
- Identify, assess, report and work with children or young people at risk of harm within the local safeguarding children partnership for Leicester, Leicestershire and Rutland policies and trust guidelines.
- Under the supervision of the Public Health Nurse, attending case conferences and contributing to strategy discussions and core groups as appropriate.
