The Care Homes In-Reach Team started as a pilot project to care homes. The teams remit has been to carry out a holistic assessment with detailed observations and assessment of behaviour, environment, and care approach in order to formulate a support plan for the individual and the care home. Our aims are to improve quality of life for people with dementia in care homes, to support care homes in delivering safe and effective individualised care and to prevent inappropriate hospital admission.
In November 2014 the team was extended to provide a mental health service to our Community Hospitals. It is now the MHSOP In-Reach Team.
Our work within Care Homes continues as before.
The team is based at the Neville Centre on the Leicester General Hospital Site and covers the City, County and Rutland. It consists of Consultant Psychiatrists, Registered Mental Health Nurses, Occupational Therapists, Health Care Support Workers, a Speech and Language Therapist and a Clinical Psychologist. It aims to respond to referrals within 72 hours.

Main duties of the job
Team members are expected to complete holistic assessments of patients, formulating plans to prevent placement breakdown or supporting smooth transfer to an alternative environment. Clinicians liaise with the teams medics at the weekly MDT. Clinicians are also required to cover duty on a rota basis, triaging new referrals and responding to contact around patients where the allocated worker is not available.
To work autonomously, taking responsibility for care, quality and clinical activity for a designated caseload of service users To contribute to care quality performance and clinical activity by providing professional leadership and guidance, clinical advice and management in relation to the key dimensions of health care quality: Patient safety The effectiveness of all health care interventions Patient experience The environment in which care is delivered To participate in the effective clinical leadership of the team. To actively contribute to the improvement of service and quality of care provision to meet the needs of service users, service users and their families. To act as role model and participate in the education, development and mentorship of other staff.
