Job summary
We are looking for an enthusiastic and experienced Occupational Therapist to join our well-developed, long-standing Community Therapy service. The service offers an urgent community response to ensure patients can be supported to stay at home to avoid hospital admission, as well as support those returning home from a hospital setting for ongoing rehabilitation to meet their full potential in their own environment.Alongside our Urgent Community Response offer we also offer community therapy to those patients with non-urgent routine therapy needs as well as therapy input into High dependency Discharge to assess beds (D2A) for patient who may have cognitive impairment, known delirium or a non-acute mental health diagnosis that require 24 hour care and 1:1 support, to provide recovery, reablement and rehabilitation, with an aim for discharge home and determine long term care needs.
Our team consists of Occupational Therapists, Physiotherapists and Technical Instructors.
Continuing professional development is actively supported via clinical supervision, annual Personal Development Plans, in-service training from a university-professional and multi-disciplinary point of view. Supervision of students is actively encouraged.

Main duties of the job
To undertake comprehensive assessments of patients with a diverse range of needs and presentations in a community setting.
To work as a specialist clinician to utilise, develop and enhance Occupational Therapy clinical and professional skills within the Service to deliver high standards of practice.
To apply the Occupational Therapy process within a Multi-Disciplinary format providing specialist assessment and treatment in clinical care pathways.
To take a lead in developing the Service. This involves promoting high quality clinical practice and ensuring professional standards are maintained in the workplace.
To deliver a specialist Occupational Therapy Service to patients within a variety of settings, requiring flexibility/adaptability in recognising the complexity of these environments to maximise their abilities and opportunities for occupational wellbeing.
