Job Overview A fantastic opportunity has arisen for a Band 7 Clinical Pharmacist to join the Community Hospital Pharmacy team at Leicestershire Partnership NHS Trust (LPT). We are seeking an enthusiastic, motivated individual with an eye for detail to join our well-established pharmacy team. We offer an innovative clinical pharmacy service that integrates technology with clinical pharmacy expertise. The Trust utilises a fully integrated electronic prescribing and medicines administration system, which includes automated controlled drug (CD) dispensing, robots, and digital CD registers. We welcome applications from pharmacists in both hospital and community settings who are eager to expand their knowledge and gain expertise in these specialised areas of healthcare.

Main duties of the job
Job Description Summary (Main duties and responsibilities):
As a Specialist Clinical Pharmacist, the post holder will be a key member of the pharmacist team, contributing to the delivery of a safe and effective pharmaceutical service to patients of the Trust. The role will include:
a) Providing a ward-based clinical pharmacy service to patients of the Trust.
b) Promoting early discharge from the hospital, reducing the length of stay, and minimising re-admissions by:
- Optimising drug therapy.
- Educating patients on the use of medicines, including self-medication schemes and adherence device assessments.
- Counselling patients on their medications prior to leave and discharge. Ensuring seamless care upon discharge from the hospital.
c) Undertaking clinical pharmacy and general departmental duties within the Trust as required.
