Job summary
We have an exciting opportunity within the team for two registered professionals to join the Learning Disability Crisis Response and Intensive Support Team as a Band 6 practitioner- x1 post at 37.5 hours a week and x1 post at 22.5 hours a week or x2 30 hours a week.
The team provides a service Monday- Sunday 8am-9pm including bank holidays and we are based at the Agnes Unit. The successful candidates will have the opportunity to discuss flexible working pattern within these hours.
You will be holding a caseload of your own and working within a specialist team of nurses, health care support workers, Occupational Therapist, Speech and Language Therapist, Psychiatry and Psychology, with input from allied health professionals- which is part of the wider Learning Disability Service.
The Learning Disability Crisis Response and Intensive Support Team provide support, clinical assessments, interventions and risk management for people with learning disabilities who are at risk of admission to an inpatient unit or at risk of losing their placement within the community.
This intensive support is provided by the model of Positive Behavioural Support and intensive support for people with mental health needs and a diagnosis of a learning disability. The service is provided to people living in the community of Leicester, Leicestershire and Rutland.
You will need a full driving license and access to a vehicle insured to use for work purposes.
Main duties of the job

- Hold a caseload of patients with complex needs living in the community, completing specialist risk assessment, risk intervention and oversight.
- Triaging new referrals from a number of sources including referral directly from patients, carers or professionals.
- Provide clinical assessment, support, planning and oversight for patients who are in the community at risk of admission and/or placement breakdown.
- Act as lead professional and completion of required assessments and record keeping.
- Working collaboratively with inpatient teams, commissioners and other case managers.
- Support the Community Learning Disability Teams where patients are presenting with high risks- through wrap around support or advice.
- Offer training and advice to carers, providers and health and social care staff specifically in relation to meeting the individuals needs and through their Positive Behavioural Support Plan.
- Provide specialist advice and support to people to enable them to remain in the community and prevent, if possible, in-patient admissions.
- Where required the service also provides direct interventions. Monitor/ support utilising our pathways including the Mental Health Pathway and Positive Behavioural Support Pathway until there is sustainable evidence that the risks are manageable/ reduced and if required handing over to LD Community teams for longer term monitoring.
