Job summary

We are looking to recruit a full, or part-time permanent band 5 Senior Family Support Practitioner, who will be part of a service that offers specialist play, emotional support and pre/post bereavement support to children/young people and their families who have life limiting/threatening medical conditions and/or complex ill health.

The ability to use your initiative and manage your time effectively is required, along with the ability to work as part of a team and wider multi-agency services.

This is an exciting opportunity to expand on existing knowledge and skills. The post holder will have a diverse and sometimes complex caseload to manage and will need to have substantial relevant experience of working with children and young people who are affected by illness in many ways.

This service is at the forefront of delivering holistic care to ensure services are wrapped around the individual and their families.

Therefore, a willingness to engage in supporting colleagues in the team and other agencies as well as working in cooperation and developing links with other statutory and independent sector agencies will be very important.

Main duties of the job

Responsible for assessing, planning, developing, implementing and evaluating programmes of evidenced-based specialist play and/or therapeutic interventions to clients/service users.

  • Assisting and contributing towards the development of an effective therapeutic play service, working with children, young people and their families who are affected by a life limiting/ threatening medical condition or complex ill health.
  • Work as a member of the Child & Family Support Service as part of the Diana Children's Community Service in the provision of Palliative Care, providing bereavement focused therapeutic support for children and young people affected by a life limiting/threatening illness.
  • Advice, support and supervise students/other professionals on the use of specialist/creative therapeutic play.
  • Work collaboratively and co-operatively with others to meet the needs of clients/service users and their families.
  • Contribute towards the design and delivery of training/teaching for a variety of allied health professionals, families, education and charitable/voluntary sector settings
