Job summary
The Memory Assessment Service are a service which is committed to delivering high quality and compassionate care for people aged 65 years old and over who have complex mental health needs, and people with suspected diagnosis of dementia. The service provides early identification of dementia or cognitive impairment by means of memory assessment, cognitive screening, diagnosis, and treatment. Part of your role will be compassionately delivering diagnosis of dementia to our patients and discuss currently available treatment options.
Clinical duties includetriaging referrals, offering advice and signposting,contributingtowards weekly MDT where your assessments will be discussed with other professionals. Carrying out specialist mental health assessments for individuals with memory impairment, imparting diagnoses and commencing treatment where appropriate.Patient assessments are face to face or virtualusing video software of which are across various LPT sites including The Bennion Centre and The Neville Centre as well as at county locations across Leicestershire.
As Team Leadmanagerial duties wouldinclude to lead, co-ordinate, organise and prioritise your own work and the work of the team on a day to day basis and ensure supervision of others and participate in education and development.
Main duties of the job
The successful candidate will be expected to perform and monitor comprehensive assessments of clients/service users needs. Plan, implement and evaluate care delivery according to changing health care needs, takingcontinuous management responsibility for the delivery of care needs of individual and groups of clients/service users. To ensure patient information is collated, monitored and fully accurate, maintaining complete and contemporaneous records in line with NMC/ professional bodies and Trust standards.
Promotingthe involvement and engagement of clients/service users and carers / relatives in the assessment, planning, delivery and evaluation of care. Workingcollaboratively and ensure maintenance of effective multi-disciplinary communication and with other professionals, agencies and stakeholders to ensure needs are met in relation to care input, support for ongoing care needs and safe discharge. The post holder will communicate highly complex information about clients/service users conditions to multi-disciplinary team colleagues, to ensure effective delivery of patient treatments.
To ensure that systems are established and maintained to ensure effective communication with clients/service users, and carers / relatives. Lead and support others to recognise, prioritise and respond appropriately to urgent and emergency situations including the assessment and management of risk.
