Job summary

We are looking for an enthusiastic and experienced Occupational Therapist to join our Community Therapy service. The service offers an urgent community response to ensure patients can be supported to stay at home to avoid hospital admission, as well as support those returning home from a hospital setting for ongoing rehabilitation to meet there full potential in their own environment. Alongside our Urgent Community Response offer we also offer community therapy to those patients with non-urgent routine therapy needs.

Successful candidates with be expected to work autonomously providing assessment and treatment/rehabilitation in peoples own homes.

We work closely with our other MDT colleagues, holding daily MDT's to support our patients in a collaborative way.

CPD is actively supported via clinical supervision, annual appraisal, in-service training from a uni-professional and multi-disciplinary point of view. Supervision of students is actively encouraged.

All our staff are fully supported by the Clinical and Operational Team.

Main duties of the job

  • To undertake comprehensive assessments of patients with a diverse range of needs and presentations in a community setting.
  • To work as a specialist clinician to utilise, develop and enhance Occupational Therapy clinical and professional skills within the Service to deliver high standards of practice.
  • To apply the Occupational Therapy process within a Multi-Disciplinary format providing specialist assessment and treatment in clinical care pathways.
  • To take a lead in developing the Service. This involves promoting high quality clinical practice and ensuring professional standards are maintained in the workplace.
  • To deliver a specialist Occupational Therapy Service to patients within a variety of settings, requiring flexibility/adaptability in recognising the complexity of these environments to maximise their abilities and opportunities for occupational wellbeing.
