We have an excellent opportunity for a Registered Mental Health Nurse, Social Worker, Occupational Therapist or Physiotherapist to join our team as a Mental Health Practitioner.

We are a Community Mental Health team providing Mental Health Services for Older People within the directorate of Adult Mental Health. We visit service users within the community in their own homes who have a functional and organic mental health illness.

Main duties of the job

You will be working within a Multi Disciplinary team comprising of Nurses, Occupational Therapists, Psychologists, Health Care Support Workers and Peer Support Workers as well as Medics and Admin staff.

You will continue to establish the good links we have with neighbourhoods and voluntary sectors, as well as Social care.

You will visit service users and assess their mental health and present that assessment back to the MDT.

You will complete:

  • Risk Assessments
  • Core Assessments
  • Care Plans
  • CPA documentations

You will also complete other relevant information, working with social services and other agencies.

Car driver is essential for this job role.
