We have a fantastic 6 months secondment opportunity for a Reconnect Engagement Worker role within the Directorate of Adult Mental Health.

The Reconnect Engagement Worker will support service users requiring assistance adjusting from prison or immigration removal centre to community life and have identified health need(s) with oversight from a registered practitioner.

The aim of the role is to improve the journey and outcome for service users as they navigate through their transition from prison or immigration removal centre to community life, to improve co-ordination and access to support services for service users, with a view to improving their wellbeing and independence.

Main duties of the job

The post holder will work within the Reconnect Service and work closely with providers of other services including the Prison Service, Probation and Turning Point to assist the service user they are working with in sourcing a solution to their care/support needs following release. This can include supporting to register/attend appointments or assessments, submit applications or referrals for support and appropriately handover to partner agencies at the end of the planned engagement.

The post holder will need to be able to provide clear advice and support to services users. They will also be required to have a good understanding of the types of support provisions available in Leicester, Leicestershire and Rutland.

Having a knowledge of the criminal justice system would be beneficial but not essential for this role.

This is a Monday - Friday 09.00-17.00 role. In addition to working in the specified LPT bases, the successful candidate will be required to work in various community locations, including Leicester, Leicestershire and Rutland prisons, Offender Management centres and homeless centres, therefore own transport will be essential for this role.
