Job summary
The Long Term Conditions Team are looking to recruit a community administrator to support our clinical teams. You will be responsible for providing admin support to our clinical teams (Heart Failure, Respiratory and Pulmonary Rehabilitation Services).
Applicants need to be proficient in a range of software programmes, such as Microsoft Office including Excel, Outlook, Power Point and Word.Applicants should be motivated and enthusiastic with excellent communication skills. Applicants need to show adaptability and flexibility whilst being team focused.
Main duties of the job
The post holder will receive and deal with telephone calls, queries, referrals and tasks from professionals and handle these as required, in order to coordinate patient care according to agreed urgent & non urgent pathways.
It is vital for the post holder to have excellent organisational skills. Good communication skills are essential as the post holder will be supporting the heart failure, respiratory and pulmonary rehabilitation team. multi-disciplinary teams. The post holder will be required to extrapolate data from various sources and should be confident in managing and collating clinical data and information.
The post holder will need to be responsible for effective communication with patients and carers as well as a wide range of health and social care professionals to ensure the service runs efficiently and effectively.
