Job summary

An exciting and unique opportunity has arisen for a Band 3 Health Care Assistant to join our community Cardio-Respiratory team in Leicestershire to play a key part in our digital cardio-respiratory virtual wards.

The virtual wards allow patients to be discharged safely from hospital and then be closely observed using remote monitoring technology in the community.

This role is part of the community cardio-respiratory team in LPT but will involve working on the cardio-respiratory wards at Glenfield Hospital to onboard and admit patients to the community cardio-respiratory virtual wards. This role also includes working in the community to complete follow up home visits for COPD patients.

Main duties of the job

This role will involve working with the multi-disciplinary team at Glenfield to help identify suitable patients for the virtual wards prior to their discharge from hospital.

The main aspect of the role will be to work with patients just before their discharge to set them up with and teach them how to use the virtual ward technology so patients ate able to use the digital system and safely enter their information and details once they are at home.

Applicants need to be motivated and enthusiastic with excellent communication skills.

Applicants also need to demonstrate they are hard working and conscientious as well as friendly and approachable.

An excellent understanding of using technology and IT is essential as this is a vital aspect of this role.

The successful applicant will working within the multi-disciplinary community cardio-respiratory team and be supported by the team leads and clinicians in the respiratory and heart failure services.

The post is permanent and will involve travel across Leicestershire.

The community Cardio-Respiratory service provides the service over 7-days a week.

The community cardio-respiratory team are currently going through an exciting period of service development and growth so this position is an excellent opportunity for someone to join the team and grow and develop alongside it.

Qualifications

Essential

  • Experience of working in a healthcare environment.
  • Level 2 qualification in functional skills Maths and English Level 3 qualification in functional skills Maths and English or required to successfully complete within six months NVQ/QCF Level in Care or equivalent qualification or equivalent relevant experience.
  • Demonstrates skills in literacy, written, verbal and an ability to effectively use electronic information and computer technology. Excellent understanding of using technology and IT with patients in a healthcare environment.
  • Own transport.
