Job summary
City East and City Central Community Mental Health Teams (CMHT)
We currently have several fantastic opportunities for experienced Mental Health Nurses to apply for Band 6 Community Psychiatry Nurse (CPN) posts in the City East CMHT and City Central CMHT.
The post holders will have a clinical case load and will be supported by the Multidisciplinary team in managing our client group.
The CMHT is part of secondary care mental health services providing assessments and treatment for adult service users, with complex mental health needs.
The teams are dynamic and constantly evolving to offer the best support and treatment to the patients that we serve. Be part of an exciting and truly multi-disciplinary approach to patient care. Work alongside social workers, occupational therapist, psychiatrists, health care assistants and peer support workers all aiming to obtain the best recovery for our patients.

Main duties of the job
As a CPN you will work collaboratively with the multi-disciplinary team (MDT) to achieve best mental health recovery for our patients.
You will be responsible for a caseload of patients of your own to support, guide and ultimately enable independence of the patients.
CPN's also conduct thorough assessments and complete clinical triages to support the team manage incoming referrals.
CPN's participate in the Duty Rota where you act as single point of contact for patient enquiries on the day.
