Job summary

Do you want to be involved in supporting service users with mental health difficulties in the community? We have an exciting opportunity for you to join the Community Enhanced Rehabilitation Team (CERT) as a deputy team lead. CERT is a new community rehab team within the enhanced rehabilitation and recovery pathway. Comprising of a multi-disciplinary team, CERT provides transitional support and mental health rehabilitation focused interventions in the community for adults with severe and enduring mental health difficulties. We work within a recovery approach, undertaking collaborative interventions with service users, families and wider networks within the community. CERT offers service users and carers a greater choice over their care and recovery, ensuring interventions are delivered in the least restrictive setting.

The Willows is currently a temporary base for CERT, the role involves blended working/working from home.

Main duties of the job

You would need to demonstrate effective assessment, planning, monitoring and evaluation of care given to service users. Providing specialised, enhanced rehabilitation and recovery interventions in the least restrictive setting. To assess, plan, implement and evaluate service users care packages in line with the Care Programme Approach in the role of Care Co-ordinator/lead professional and to undertake the role of lead clinician for service users who are under a Community Treatment Order (CTO). To participate in the effective clinical leadership of the team. To act as role model and participate in the education, development and mentorship of other staff.
