Job summary

We have a permanent position available for a Junior Pharmacist Band 6 (37.5hrs) to join the Pharmacy team at Leicestershire Partnership NHS Trust (LPT).

We would welcome applications from newly qualified pharmacists or those wishing to start a career in hospital pharmacy. You will rotate through different areas of pharmacy and specialties within the Trust, gaining foundation knowledge of hospital pharmacy.

We are looking for an enthusiastic, motivated individual with an eye for detail to join a well-established pharmacy team.

We provide an innovative clinical pharmacy service, combining technology and clinical pharmacy expertise.

Experience essential to this role:

Excellent oral, written, interpersonal and communication skills and the ability to engage with NHS staff at all levels

Ability to prioritise and work on several issues concurrently

Good general knowledge of primary care issues

Sound clinical knowledge: clinical knowledge relating to medication use in secondary care would be particularly useful

An analytical approach to problem solving by applying clinical expertise alongside shared decision making

The successful candidate should have good computer skills, demonstrate they can work accurately, have excellent communication skills and be proactive. They need to be able to work as part of a team, work in an organised manner, and have a willingness to learn.

Hours of work will be Monday to Friday 8.30am 4.30pm / 9.30am 5.30pm.

Main duties of the job

As a specialist Clinical Pharmacist, the post holder will be a key member of the pharmacist team and in the delivery of a safe and effective pharmaceutical service to patients of the Trust. The role will include:

a) The provision of a ward based clinical pharmacy service to patients of the Trust.

b) To promote early discharge from hospital, reduce length of stay and reduce admissions by:

  • Optimising drug therapy.
  • Educating patients in the use of medicines, including self- medication schemes and adherence device assessments.
  • Counselling patients on their medicines pre leave and pre-discharge.
  • Promoting seamless care on discharge from hospital.

c) To undertake clinical pharmacy and general departmental duties within the Trust as required.
