Job summary
Bank Band 6 Community Learning Disability Nurse.
We work with individuals and their carers with Learning Disabilities that have health needs relating to their mental health, physical health, behavioural needs and other interventions to promote healthy lifestyle and well-being.
The Community Learning Disability Nurses work closely with SALT, OT, Psychology, Physiotherapy, LD Outreach, LD Forensic and Psychiatry teams to provide holistic and collaborative care for our patients.
The team work hard to ensure that patients living in the community are able to manage their mental and physical health, ensuring they are able to access the necessary level of support from all health care providers and services.

Our core hours are from 08:30-16:30 Mon-Fri- we can offer flexibility within these times, with half-days or less considered for the right candidates.
This exciting opportunity to join our well established MDT has arisen due to career progression.
We are looking to recruit nurses who are passionate about meeting the holistic health needs of people with Learning Disabilities and addressing health and inequalities that are prevalent within this cohort of patients.
Own car is essential for this role as you will be travelling to visit patients, public transport is not suitable for this role.
Main duties of the job
- Responsible for assessing, planning, implementing and evaluating evidenced-based care to a individuals with a diagnosis of LD, ASD and behaviours that may be challenging those that care for our patients.
- Act as a clinical and professional role model, assessing and supporting students and learners on placement within the clinical area.
- The role can involve dealing with and supporting highly distressed people. Your skills and experience as highlighted in the Person and Job specifications attached to this vacancy will be key.
- You will be required to complete physical health checks -depot injections -metabolic monitoring of patients on anti-psychotic medication, and provide support to carers and families.
- There will be opportunities to contribute to wider team quality improvements.
- To support the management of the caseload including allocation and waiting lists.
- You will work closely with the Band 7 and deputise as and where required.
There will be a variety of settings and opportunities for bank staff across the whole of the nursing community specialist nursing offer.
