Job summary
Trusted Assessors were introduced to Leicestershire Partnership Trust 2
years ago, with an aim to support and facilitate discharges from Mental Health
Services for Older People (MHSOP) to residential and nursing care home
providers. The successful applicant will complete a team of two, joining an
experienced Trusted Assessor that has been involved since the piloting of the
role. The post holder will be the expert in Trusted Assessment and will nurture
and develop trusting relationships between Leicestershire Partnership Trust
services, social services and care homes, supporting patients and families in
the process.
The post looks to strengthen relationships between our community providers
and the acute setting, to ensure that discharges are high quality, person
centred and ultimately successful, resulting in fewer failed discharges. The
Trusted Assessor will be required to identify and problem-solve upcoming
barriers to discharge and demonstrate the abilities to challenge senior
professionals where indicated. The Trusted Assessor will be required to work
flexibly and collaboratively within multiple MDTs, to proactively identify
patients needs and to ensure least restrictive and holistic approaches to care
are facilitated.
The post holder will be
working as part of the patient flow team, leading meetings as required and
consolidating information from all professionals, patients and family members
to assure a safe and timely discharge.
Main duties of the job
The post holder will need to be able to form relationships with internal
and external stakeholders, enabling the post holder to work collaboratively to
ensure a timely discharge.
To screen, gather information assess and plan care for adults. To
include the consideration of any communication and cultural needs.
Pre-discharge assessment will be completed for individuals that are returning
to a residential / nursing home, those new to a care home, or those whose needs
have changed and need to move from residential care to nursing care.
To identify and investigate appropriate resources relating to the
individual needs and to work with fast track individuals to ensure discharge is
as rapid as possible.
To interpret complex information and produce high quality assessment
documents. The post holder will have good analytical skills, able to
assimilate, interpret and present complex information from a range of varied
sources to provide a person-centred assessment report.
To attend Discharge meetings and Multi-disciplinary Team meetings, to
pro-actively identify individuals suitable for services. This could also
include arranging any practical needs to ensure the individual can return to
the care home safely from hospital.
