Job summary
An exciting opportunity has arisen to join the South Leicestershire and Harborough Healthy Together Team within school nursing. Our teams work within Public Health Nursing to deliver the Healthy Child Programme.

In the County we are a school 5-11 service. Our teams are busy, high-achieving, supportive and dynamic in the delivery within the local community.
Public Health Nursing is a discipline of its own, which seeks to promote and protect the health of school-aged children and young people, through early intervention and health promotion in variety of settings.

Healthy Together offer a range of services to meet the varying needs of the local population, which includes our well-developed digital offer. Main duties of the job
The Healthy Child Programme Practitioner (HCPP) will undertake health promotion activities including supportive behaviour management strategies with Universal and Universal plus Partnership plus children and their families, delegated by the Public Health Nurse as outlined in the Standard Operating Guidelines (SOG) for Leicestershire and Rutland and associated pathways.
The post holder will be responsible for planning, delivering and evaluating a range of public health activities and packages of care on a 1:1 basis or in groups for example;
- Defined packages of care for sleep, behaviour
- Review assessments for children with self-harm and emotional health issues where risk is assessed to be acceptable by PHN
- Healthy Bladder/Bowel workshop delivery
- Healthy Bladder/Bowel Reviews
- Delivery of parent/carer public health information sessions
- Lead and coordinate Public Health programmes for identified neighbourhoods
- Promotion of nutrition and healthy weight and obesity prevention in group setting or 121 follow up advice
- Support National Childhood Measurement Programme (NCMP)
