Job summary

An exciting opportunity has arisen for a Dynamic Support Pathway Lead to join the LDA Collaborative working across Leicester, Leicestershire and Rutland. Strategically leading & governing the Dynamic Support Pathway for CYP and Adults with LDA, improving admission avoidance processes, identifying solutions & negotiating and resolving barriers.

The LDA Collaborative is committed to improving the lives of children, young people and adults with a learning disability & autism.

This post will lead delivery of the Dynamic Support Pathway across Health & Social Care and Education systems in Leicester, Leicestershire & Rutland.

We are looking for someone with a proven track record of cross organisational partnership working who will maintain the co-ordination function of the pathway and ensure high levels of multi-agency engagement.

We are looking for an experienced professional who will be able to provide oversight & guidance to clinical decision making required, follow up processes involved ensuring tasks agreed within this pathway are delivered & to act as an advocate for the families, carers and individuals.

The post holder will have experience of monitoring delivery & performance against targets, leading the collation and evaluation of system wide feedback pertaining to this pathway in order to deliver on-going review & continuous improvement.

We maintain a best practice approach to engagement and co-production in any future development of this pathway.

Main duties of the job

Some of the duties in this role include:

Producing high quality data analysis, plans and reports on behalf of the LDA Collaborative to present at system-level and NHSE regional meetings.

Providing operational management & clinical leadership to designated areas/department of responsibility within the LDA services.

Work alongside clinical teams providing assurance about the quality and delivery of clinical practice.

Effectively develop & cascade policies guidance and pathways across the service and to advise on the require interpretation/implementation within practice.

Establish sound & effective relationships with service stakeholders and other service providers to develop integrated approaches to care provision within a Multi-agency system.
