Job summary
As a Diana Acute Nurse, you will work as an autonomous practitioner in assessing, planning, implementing and evaluating specialist nursing care for a designated caseload of children/ young people including those with life limiting/threatening needs and technology dependent in a variety of community settings.
You will be expected to facilitate empowerment and partnership working with children/ young people and their families and multi-agency services.
You will be an effective team player, but also be able to plan and organise your own workload, possess excellent communication skills, and an ability to teach families and health professionals
You will be the advocate for the child/ young person and their family.
As part of this role you will be expected to co-ordinate the provision of appropriately managed packages of care for terminally ill children/ young people in hospital and community by being involved with 24/7 on-call system.
Main duties of the job
To work autonomously taking responsibility for care, quality and clinical activity for a designated caseload/team
To contribute to care quality performance and clinical activity by providing professional nursing leadership and guidance, clinical advice and management in relation to the key dimensions of health care quality: Patient safety, The effectiveness of all health care interventions, Patient experience and The environment in which care is delivered
To participate in the effective clinical leadership and management of the team.
To actively contribute to the improvement of service and quality of care provision to meet the needs of patients, service users and their families.
To act as role model, mentoring, teaching, assessing and supporting students and learners on placement and participate in the education, development and mentorship of other staff.
To work in collaboration with other senior nurses across LPT as appropriate and promote the role of their speciality nursing to all stakeholders.To foster a culture of networking in an open and transparent approach with internal and external partnerships and between all stakeholders.
Responsible for the assessment, planning, implementation and evaluation of programmes of evidenced based care to a group of patients according to changing healthcare needs and varying levels of complexity.
