Job summary

Are you an experienced Healthcare Assistant/Support Worker who has worked in a community or other healthcare setting?

Do you have an NVQ level 3 or equivalent in health and social care?

As an ageing well accelerator site LPT have been awarded extra funding to increase our health care support worker team. This is an exciting opportunity for experienced health care support workers to become part of our Community Nursing Teams based in Braunstone, Barrow upon Soar, Rutland, Syston, Melton, Enderby, Coalville, Hinckley and Market Harborough. The teams provide core community nursing, rapid response nursing and Home First nursing interventions to patients living in their own homes, residential homes and in local clinics.

We work in an integrated way with health and care providers across the whole system in Leicester, Leicestershire and Rutland to ensure that care is joined up and that whenever we can, we strive to care for patients at home, helping them to maintain independence and ensuring that high quality care is delivered in an efficient and holistic manner.

We are working hard to continually improve our outcomes for patients and our experience for staff in delivering care and have new investment to improve our capacity to keep developing.

Main duties of the job

Demonstrate your passion and ability to provide care for patients experiencing a wide range of conditions.

Support our patients and work with people in an asset-based way that really enables them to live their optimum life, being self-caring and independent with their health needs where possible.

Work collaboratively, efficiently and creatively with multi professional colleagues and partners with a focus on ‘HomeFirst’ helping patients to avoid hospital admissions and where they do need a hospital stay facilitating timely and well supported discharge back home.

The principle role of the Senior Healthcare Assistant is to assist the healthcare team in the provision and delivery of programmes of patient care under the indirect supervision of registered practitioners. This includes: Taking physical or psychological measurements from named clients/service users as delegated by the team; 

 Supporting clients/service users in maintaining their health and wellbeing; 

 Undertaking specified and delegated clinical and therapeutic activities; 

 Under the supervision of healthcare professionals assist the healthcare team with the planning, organising and provision of clinical care; 

 Ensuring high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals; 

 Assisting Registered Professionals in the implementation and evaluation of individualised care plans; 

 Accurately recording patient information; 

 Provides support and supervision to other colleagues and learners as appropriate.
