Job summary
This post is essential for the delivery of a modern, specialist mental health service across the primary-secondary care interface in the health community. The post holder will use their experience, skills and initiative to develop, co-ordinate and provide a high quality clinical pharmacy service to community based patients and their carers within Leicestershire Partnership NHS Trust (LPT), in one or more allocated localities of the organisation.
The post holder will be an expert in psychiatric therapeutics and provide a specialist referral point for all health and social care professionals working within the community teams. They will be a core member of multidisciplinary team meetings and undertake clinical audit and quality improvement initiatives where required to improve services.
The post holder will be expected to have their own client caseload and delivery quality-driven key performance indicators in line with Pharmacy and Trust priorities.
Main duties of the job
Supporting the monitoring and management of patients with serious mental health conditions and co-existent physical healthcare issues
Running general out-patient clinics as a prescriber, holding their own caseload if required
Providing specialist mental health advice to GPs to potentially avoid referrals
Running specialist out-patient clinics e.g. for patients withdrawing from benzodiazepines or antidepressants
Supporting assessments
Promoting shared decision making with patients at the point of drug initiation
Reviewing patients response and toleration of medication and identifying those where the balance of risk and benefit no longer supports continued prescribing
Ensuring the requirements of MHRA alerts are met e.g. use of Sodium Valproate in women of child-bearing age
Identifying all patients who would benefit from Clozapine, thus speeding up access to that medication and thereby improving response rates
In-depth medication reviews as requested by other team members when the appropriate future medication treatment plan is unclear
Running support groups on medication for patients
Providing high-level medicines training for other healthcare professionals
