Job summary
An exciting opportunity has arisen for a motivated and enthusiastic Band 5 Registered Professional (Mental Health Nurses, Social Workers and Occupational Therapists) to join the Adult Crisis Resolution and Home Treatment Team based at the Bradgate Mental Health Unit.
The key skills for this role will be the ability to demonstrate excellent ongoing assessment, problem solving and risk assessment skills.
The successful candidate will demonstrate the ability to work independently.

Main duties of the job
The Role of a band 5 Mental Health Practitioner within the Crisis Resolution and Home Treatment is to:
- Co-ordinate and provide specialist acute crisis psychiatric interventions and expertise to services users in crisis 24/7 per 52 weeks per year within their own home/residence
- To facilitate early discharge (Step Down) where appropriate and provide an effective viable alternative to hospital admission for clients in crisis by developing and individualised approach to the practical and functional needs of the service user.
- The role is dynamic and requires specialist assessment of risk
- The Adult Crisis Resolution and Home Treatment team is professional and diverse team to work with and will provide you with lots of experience across the acute sector
