Job responsibilities
- To support and implement care that meets the mental, physical, spiritual,psychological and social needs of the patient, being sensitive to age, culture,race, gender, ethnicity, sexuality and disability.
- Take a lead in facilitating the day to day functioning of the Clozapine Clinic, withsupport and guidance from qualified professionals such as the nursing team,consultants and pharmacists.
- To monitor the effects and side effects of Clozapine medication in the community,with guidance from relevant professionals, such as the nursing team, consultantsand pharmacists.
- To administer intramuscular injections and monitor the effects of these, with theknowledge of contraindications and side effects, being able to relay accurateinformation to patients in a format that they are able to understand in accordancewith SOP0031.AFC 06/08/19 JME-335-19 Community Mental Health Nurse Band 5
- To undertake venepuncture in the context of Clozapine monitoring, after relevanttraining and ongoing guidance.
- To undertake physical health checks, interpret results and respond to and reportconcerns appropriately. (No moving and handling required).
- Assessment, Care planning and Risk Assessment in line with Trust Policy.
- To be able to identify and respond to concerns appropriately.
- The ability to make a judgement about a patients mental health or wellbeingneeds and act appropriately to manage needs / risks. (For example, supportingself-care needs by helping a patient to organise their prescribed medication into adosette box, or reporting a concern / risk to a qualified member of staff).
- To maintain the safe storage of medical equipment within the clinical setting, inline with Trust Policy.
- To monitor clinical stock supplies levels and support in the maintenance ofadequate stock levels.
- To take responsibility for maintaining the safe use of clinical equipment usedwithin the role.
- To contribute to the provision of a safe clinical environment and promote thedignity of patients.
